For your convenience Alaska Surgery Center is now offering online pre-registration for your surgery.  Registration is designed to be quick and easy and completing the form should take you 10-15 minutes.  Please have your insurance information handy. 

Online registration requires 2 business days to process.  Please call 907-550-6100 to register by phone if your surgery is less than 2 business days away. 
 
Please have these documents filled out and return on the patient's date of surgery.
